Dear Latchkey Parents,
We would like to take this time to walk through the enrollment procedures:
ENROLLMENT FORMS: In order to enroll your child in the program you need to complete an Enrollment Form, Over The Counter Medical Form, and a Health Report Form. If you were enrolled in the Program last year then you are able to use the existing forms from the previous year. If there are any changes in your paperwork at anytime please inform the office. EVERYONE NEEDS TO COMPLETE THE BOTTOM PORTION AND RETURN TO SIMON SAYS. Please remember to look for the sign up sheets for half days and whole days. We need to be able to staff appropriately, so please try to sign up a month in advance. We will be providing lunch for the children on these days. We would appreciate it if you would not bring your childs lunch, due to the fairness to the other children. It is very important that your child bring a change of clothing, the program tends to get messy at times!! For those that attended the Summer Camp Program it was a huge SUCCESS!! Thanks for sharing your summer with us and we hope to see back next year. If you have any questions about the program please ask us or call the office at 842-3435 and speak to Michele Snyder. Welcome to the Simon Says Latchkey Program. We are looking forward to an exciting and fun year. We have a fairly new staff who are anxious to begin. Please help us in guiding them through the first weeks of Latchkey. Please remember that the children are to be picked up ONLY at the door across the double doors of Simon Says. We are not to use the front school doors.
